I am restoring a 1930 Chevy Sedan. Does anyone know why there would be an extra hose coming off of the outlet elbow of the radiator. Picture attached. I never saw the radiator when it was in the car, so I don't know where it would have been connected.

I saw a picture of a different engine that had an extra hose running to the thermostat, but I haven't seen that on most other engines. Also, I've read about people adding overflow tanks.

Would it be okay to just connect the inlet and outlet hoses and remove the extra hose coming off of the elbow (with a cap of course)

The red hose is a heater hose. You can cap it or replace the entire elbow pipe. They are available from the Filling Station.

Would the heater have been an after market add on? How were the cars originally heated?

think some were dealer options (Chevrolet Versions), other were aftermarket.
most of them had NO heat like my 1929 !!
